Option 2: Manually file the application at any of the Offices or Stock Points of DXN. If you need our assistance, please send the completed form to our address by courier / post. We will register at DXN and send you the details. Procedure for preparing the papers is given below.

STEP 1: Take a print out of all the FOUR pages of the 'Distributor Application and Agreement Form’: Click here to view / print.
STEP 2: Procedure to fill-in the form:

• Fill in the form in BLOCK letters only: Provide maximum information. Enclosures as required / mentioned in the application form.
• Compulsory fields to be filled in (without which the registration will not be processed): Name, Date of Birth, Sex, Marital status, Name of the Spouse (if applicable), Name of the Beneficiary, Bank details, PAN, Phone numbers, Email ID, Postal address and ID proof / Address proof.
• Affix a Passport size Photograph on the form - First page, Top right and then sign across.
• Applicant's Signature : (at FOUR places): At the bottom of the first page at 'Applicant's Signature'. At the bottom, blank space of the 2nd and 3rd pages. At the bottom of the 4th page at ‘Applicant’s Signature’.
• Attach a copy of ID proof (Containing the Picture, Signature and Complete Address of the Holder): e.g. Passport, Ration Card, DL, Aadhar, Voters ID. Sign on the copy, as your endorsement / confirmation. Cancelled Cheque is required as confirmation of the Bank.

STEP 3: Send the whole set to us by Speed Post / Courier.
